What is the Caregiver Support Program?

The Caregiver support Program is a case management program that assists Sacramento County caregivers. The Caregiver can be a friend, family member, neighbor, child, or spouse; any age, gender or ethnicity. Our goal is to help caregivers access coummunity resources through case management servies, information and referral, and ongoing support

What do we do?

The caregiver will work with a case manager to develop a cre plan and access community resources. Case management services assist with accessing necessary goods and services, for example, respite, adult day health care, home safety modification, and incontinent supplies. If the client does not require case management services, we can assist with information and referral resources only.

The Caregiver Support Program is a resource dedicated to imporving the well being of caregivers. Care giving can be a joy, but also stressful and exhausting. If we can promote home and community based care, through safety and supportive services, our mission is accomplished.

Who is eligible for the Caregiver Support Program?

Caregivers who are living in Sacramento County, caring for someone 60 years of age or older, may be eligible for services. The care receiver is often frail, or experiencing cognitive impairment requiring supervision. Priority is given to caregivers who are low income, socially isolated and requiring the greatest need.

How do you make a referral?

Anyone is welcome to make a Caregiver Support Program referral.

The caregiver is considered the client in this program. The following is necessary at the time of the referral: name, address, contact person, date of birth, information about the situation, and the level of functioning of the care receiver.

Other

There is no charge for services.

A client does not have to be a patient of the UC Davis Health system to receive services.

We are funded under a contract with Area 4 Agency on Aging

Calendar of Patient Education Classes

Calendar of Patient Education Classes with a brief description of each class, is now accessible from the UCDHS web site at http://wellness.ucdavis.edu. Patients may register for classes on-line, or by calling the Center for Nursing Education at 734-9797. The calendar is updated semi-annually.
